The most difficult in the world ? EU-189 has already been activated, while there are several other unreferenced islands. I wouldn’t define it that way.

Places like Navassa and Desecheo may be more difficult to activate due to politics, but I reckon there is no more difficult IOTA to activate due to difficulty of access than Rockall. If yourself or anyone else knows of one (island) I am sure Andy Strangeway would love to know about it : – )
The most difficult IOTA to activate is obviously any one of the (probably several hundred) islands that are yet to be activated. If they were not more difficult to activate than EU-189 then these unactivated islands would have already been activated
